Nebraska State Immunization
Information System
Family Practice is currently enrolled in the Nebraska State Immunization Information System (NESIIS). This program allows parents, schools, and medical facilities to access a child’s immunization records. Parents may log on by entering their child’s social security number, name and date of birth. LOG ON.
For over 40 years, Family Practice of Grand Island has been providing exceptional care to you and your family through all stages of life. Through the years, health care has evolved so that many services have become more specialized, and the way we provide care to our patients, as family physicians, has also evolved. With that being said, Family Practice of Grand Island has made the decision to discontinue providing obstetric services at the end of 2023. Our doctors will no longer deliver babies but will still provide women’s care including family planning and contraceptive care, as well as preventative gynecologic care through all stages of life. Family Practice will remain committed to caring for patients from newborns to geriatrics. We appreciate your loyalty to us and we promise to stay committed to the greater Grand Island area for your family’s health care needs.
